There are a total of 33 Codex Entries in the game. Some of them are obtained automatically as you progress through the story, while others require you to ask specific questions to certain characters or interact with various objects. You can check and read the Codex Entries in the third tab of your journal.
If you collect all 33 Codex Entries, you’ll unlock the 'Monica Would Be Proud' achievement.
