Season 4 has been live for less than two weeks and the bug list is already stacking up. From a grappling hook that traps you mid-air in Warzone to an optic that just got yanked out of the Gunsmith entirely, there is a lot going on right now that is actively affecting how the game plays. Some of these have fixes scheduled. Some are still under investigation. A few have no timeline at all.

At the same time, Season 4 Reloaded drops on June 25, and Treyarch is already teasing the new Zombies map, a remastered multiplayer map, and a new Endgame operation. So the question right now is not just what is broken but whether any of it gets cleaned up before the mid-season push hits.

Here is every confirmed bug, every scheduled fix, and everything officially revealed for the June 25 update.

The Greaves Optic Is Gone and Your Loadouts Are Broken Because of It

The biggest change affecting loadout builders right now is the Greaves Ultra Zoom long-range optic being fully pulled from Black Ops 7 and Warzone. If you had it equipped, you will notice it disappears every time you load into a match. If you try to build or edit a class with it in the Gunsmith, it gets stripped out the moment you exit.

There is no confirmed fix date yet. The COD Updates account confirmed the team is investigating an issue tied to the attachment, which is why it was restricted rather than just patched. Sniper and LMG players should rebuild their secondary attachment slots around the optic slot being empty for now.

Every Bug Currently Active in Black Ops 7 Season 4

These are confirmed issues that are either under investigation or have fixes scheduled, pulled directly from the official Known Issues tracker and COD Updates posts.Global Issues Affecting Both Titles

  • Vast Convergence Foregrip broken on all Season 4 weapons. This is not limited to one weapon class. The Red Well Shade Suppressor got a fix, but the Convergence Foregrip is still broken across every new Season 4 gun. No confirmed fix date.

  • Players returned to lobby when highlighting the Fang Hover Point ELO. Still under investigation.

  • Gunsmith button prompt missing for melee weapons. A fix is in progress.

Warzone-Specific Bugs

  • Grappling hook stuck animation. Players are being frozen in the grapple animation with no way out. The team confirmed they are investigating.

  • Graphical lighting glitch on Rebirth Island. Bright light flashes are appearing above certain POIs, mostly reported around the Chem area.

  • Grapple hooks not connecting with surfaces consistently. Separate from the stuck animation, grapples are sometimes not latching at all.

  • Legacy weapon camos displaying incorrectly. Older camos from previous seasons are broken in Warzone.

  • Momentum perk outlines still present. The perk was disabled to stop the unintended outlines, but the visual fix to make it display properly again has not shipped yet.

Multiplayer Issues

  • Care package missing scorestreak icon for Jackpot Jinx Overclock users. The reward icon above dropped care packages is not showing for players who rolled via Jackpot Jinx. A fix is scheduled.

  • Theater Mode crashes when loading a recent match. Under investigation.

Zombies Issues

  • Music Box Relic on Totenreich blocks pickups. Enabling the Music Box Relic on Totenreich prevents you from picking up zombies while in the Disciple Mutant Injection.

  • Music Box bug on Ashes of the Damned. A separate Music Box issue also affects Ashes of the Damned.

Endgame Issues

  • Arrow removed from ground stability issue. Still being investigated.

  • Crash when leaning out of a vehicle. Pressing the lean-in and climb-to-roof buttons at the same time in a vehicle can crash the game.

Warzone Ranked Just Got Three Map Rotation, Core Resurgence Is Next

Warzone Ranked Play quietly added three-map rotation for Resurgence, cycling through Rebirth Island, Fortune’s Keep, and Haven’s Hollow. Before this change, ranked Resurgence was on a single map loop.

Based on a post from the COD Updates account, the development team confirmed that three-map rotation is also coming to core Resurgence Quads this week. Since Season 4 launched, Fortune’s Keep Quads has been the only core Resurgence playlist. That changes with this week’s playlist update, expected Thursday or Friday depending on when the Warzone update goes live.

If you have been grinding Resurgence Quads and felt like Fortune’s Keep was getting stale, this rotation restores Rebirth Island and Haven’s Hollow to the loop.

New Aftermarket Part Coming Thursday With Weekly Challenges

The weekly challenge update drops Thursday, June 19 as usual. Along with a fresh batch of challenges, a new Aftermarket Part weapon mod is expected. Based on the order listed in the Season 4 blog, the next reward in line is the XR3 Ion Vulcan Minigun underbarrel conversion. This attachment completely transforms the base weapon’s firing setup.

There is a possibility it could instead be the DS20 Mirage Dual Fire Kit or the V-Lox 5.7 Carbine Chassis, as the details for the upcoming reward are still listed as redacted. Regardless of which one ships, a new Aftermarket Part is confirmed for Thursday.

This Thursday’s update also marks the point where seasonal camos hit the midseason mark, since Reloaded lands the following week.

Season 4 Reloaded Hits June 25: Kowakujo, the AN-94, and More

Season 4 Reloaded officially launches June 25, 2026. This is the confirmed date from both Treyarch and the COD Updates tracking account. Here is what is confirmed to arrive.

Kowakujo Zombies Map

The centerpiece of Season 4 Reloaded is Kowakujo, a new round-based Zombies map set inside a feudal Japanese castle positioned near an active volcano. Treyarch has already begun releasing teaser clips showing off areas of the map, including the exterior zones around the volcano. The map focuses on the character Takeo and introduces a new Wonder Weapon, new enemy types, and new GobbleGums.

The setting was teased at the end of the Totenreich Easter egg, where “the place of fire” was referenced. Based on the footage released so far, the map has a very distinct atmosphere compared to Totenreich and Ashes of the Damned.

AN-94 Assault Rifle

The AN-94 is confirmed as an event reward at Reloaded. This is the iconic assault rifle where the first two rounds fire at a burst rate before switching to full auto, a mechanic that originally appeared in Black Ops 2. It should be particularly relevant for players who like high-skill-ceiling weapons with early-shot precision.

Launch Map Returns to Multiplayer

The Launch map from the original Black Ops is coming back as a remastered 6v6 map at Season 4 Reloaded. It joins the existing Season 4 map pool.

Endgame: Operation King Killer

For Endgame players, Operation King Killer starts at Reloaded, continuing the Guild storyline in Avalon.

Season 4 Reloaded Blog and Roadmap

A full Season 4 Reloaded roadmap blog is expected either later this week or the Monday before the June 25 update. Treyarch has been releasing teasers throughout the weekend, so the official blog with the full content breakdown should come soon. Keep an eye on the Call of Duty blog and the COD Updates account.

Black Ops Cold War Also Got a Patch

For anyone still loading up Cold War, the game received a title update on PlayStation 5 that disabled select graphics settings while the team investigated a crash triggered by a previous update. A fix is in progress for that crash. It is a niche situation but worth flagging for PS5 players who have been running into unexpected crashes in Cold War since the recent platform update.
